I think I’m going to have to give a hint for question #1: It was on probably the most obscure episode of The Brady Bunch ever.

You’re batting 1000 on all but question #2. The Kellys, Mrs. Payne the racist played by Molly Dodd, who was also on the episode where Marsha got braces. I’m still looking for the actor who played Mr. Kelly, and what was special, from a network point of view, about that episode. Bonus points if you can use the industry term.
To further explain the next link in the chain, there was a similar show to the Brady Bunch that was on during the same years. Their own version of Marsha got braces. Who was it, and on what show?

Lori Partridge, played by Susan Dey on The Partridge Family. She got braces in Season 1.

There seems to be some disagreement on this. I’ve seen interviews with Jeremy who says his family moved back East and didn’t want him living without them in LA. But Shirley Jones says on one of the interviews on my Season 1 DVD set that he just couldn’t keep up with the others acting-wise. She said that Susan Crough, who was younger than Jeremy, was an old hand, but Jeremy was doing his first acting and wasn’t really very good. (This bit is paraphrased from memory, but it’s the gist.) Personally, I thought he was fine. Perhaps he took more takes to get a good performance, which wouldn’t show to us at home.

He later did a little movie called Corvette Summer. Heh. Just kidding. He was Luke Skywalker!
Before someone else is forced to do it, let me correct myself. Hamill has two 'l’s and Laurie Partridge’s name wasn’t spelled ‘Lori.’
