I haven’t found anything definitive saying that he is only a member of the bar in those two jurisdictions, but I also can’t find anything saying he is licensed anywhere else, so I believe you are correct.
Same here. Given his career, I would assume he only sought admittance to those two, and appeared Pro Hac Vice everywhere else. NY and DC would be the bulk of his legal work. However, even if there is another state out there, they would almost certainly do the same. If you get disbarred one place, other states almost always follow suit.

You know I’ve wondered about that. What sort of hair coloring turns liquid and runs down? I used to color my own hair; nothing I ever used would do that. What did he use? Kids’ water colors, shoe polish, indian ink?

Enlighten me please? I don’t know what that is. Though given who he supports the involvement of a sharpie in some way isn’t too surprising.
You can use a permanent marker to color your hair, although it’s not “permanent” but rather lasts for hours, and can really damage your hair.
You also don’t need to scribble on your hair or anything, you can cut open the marker to “harvest” the ink and use that (likely mixed with something else).
Just, don’t do it, it’s a bad idea.
Apparently though, it’s not that unusual for conventional hair dye to “leak” if it’s a bad quality or applied incorrectly. And I don’t see either of those situations as being unlikely for the esteemed Mr. Giuliani.
