I’ve decided to modify my answer to #3. I considered both forms of the answer before providing the first one, but I now think it would be better to say that it’s a one-word name that is always preceded by a title or honorific.

#1 was Greta Garbo. #2 was correct. #3 is either Gus or Gus-Gus, depending on your source, so, correct.
