According to TV Tome, “Joan”, the name Buffy chooses for herself in Tabula Rasa, is also the “real” name of Chanterelle, the wanna-be vamp goth chick from season one who’s name was also Lily in LA in the premiere of season 2, who took Buffy’s middle name Anne when she got away from the mission/cult thing.
IIRC, Buffy just picked the nametag out of a basket of name tags, figuring that one was as good as the next.
Lily showed up in a couple of episodes of Angel as well, running a shelter.
IMDB has her listed as “Chanterelle/Joan” in her first appearance in Season 2’s Lie to Me. Although, I don’t believe she mentioned her real name in that episode.
According to a transcript I just browsed, Chanterelle’s real name isn’t mentioned in “Lie to Me.” (Although it is revealed that Diego, the dork in the cape, has the real name of Marvin.)
I’m with Fiver that there was a lot of potential in Anne (I’m calling her that because we know she used it for at least six years). In the episode Anne, when Buffy asks her what they call her at home she totally shuts down. A very nice little character moment – in that one second you can imagine why she’s living on the streets and why she desparately doesn’t want to be the person she used to be. The way you see her as a street person in Buffy and then a couple years later she’s gotten her stuff together and is helping people in Angel is a very optimistic story – and since there’s no direct mention of Anne’s past on Angel, it’s pretty subtle. (It reminds me a lot of Audrey Murray from The Invisibles comic.)
Hmm…at least I know I’m not going crazy. I couldn’t remember that for the life of me actually being said in any ep. Wonder if it was mentioned in the script and then cut. IMDB often has info like that that came from the studio (or someone with script access) that never actually makes it onto the screen.
 I have no idea what the answer in. All I know is I just happened upon a thread title that just happened to include my mother’s name, the name she gave to the cat I gave her, and my name. And frankly, it’s freaking me out a little. (Should the three of us expect a “Lily” along and minute now?)
I did not make the connection between Chanterelle/Anne on Buffy, and the character on Angel. I’ll have to check it when the last season of Angel is released on DVD. What a nice continuation and then ending for the character.
