Glass bottle, very chilled.
I love crushed ice but we have cubed. First I use My Glass (tall, red, plastic) and fill it almost to the top with ice. Pour in some dollar store coke.
So cold it’ll give you a brain-freeze. 
If I’m just drinking it, I like a 12oz can. It’s a good-sized amount. I don’t like it too cold and I usually keep soda out on a shelf and drink it room temperature rather than refrigerate it.
When I’m driving, I like a 20z bottle. Then I can take a drink, recap it, and space it out for about an hour.
My preferred version of Coke is Cherry Zero.
Straight from the can or bottle, chilled if possible, but not essential.
I never use ice cubes unless I’m drinking something mixed with hard alcohol or if it’s really hot out.
Diet, chilled but no ice, in a glass mixed with Jack. Wedge of citrus optional.
(Non-diet cola is gross.)
Back when I was drinking Mountain Dew* I’d try to cut down by drinking half of one, but since it only tasted good straight from the can, I’d dump the rest of it down the drain unless someone wanted the rest of it.
*(I’ve moved to Sugar Free Red Bull and don’t want to waste $1 by throwing away half of one, and at any rate if I wanted to have less caffeine at that particular moment I can just drink weak tea.)
In a nice glass, with rum. Hold the cola.
I don’t often drink it, but when I do it’s regular Coke in a glass with plenty of ice. All the better if I can find the stuff made with real sugar.
I never mix soda/pop with booze. It ruins both, imo.
Diet Pepsi with Lime, from a can straight out of the refrigerator.
That said, I’m not too picky, as long as it’s diet something. My most common consumption method is Diet Coke from a fountain in a plastic cup with ice, with a straw, because that’s how I do it in the cafeteria at work.
I switched to diet seven or eight years ago and the ‘normal’ versions taste a little bit off now. Except for the cane sugar stuff (a.k.a. Mexican Coke), which is still a pretty delicious treat.
If it’s from a can, I’ll dent the can with my thumb right under the opening. That way, if I’m driving, I don’t have to look to see how the can is oriented. Also, it marks it as being “my” can.
If it’s from a bottle, I love to drop a small bag of peanuts or cashews into the bottle. That way, I can get a swig and some nuts to chew on at the same time.
At home I drink straight from the can, no straw. When out I prefer a 20 oz. plastic bottle because they travel better.
I drink cola a few times a year, in one of two forms:

In a glass, with or without ice, with rum, bourbon, or rye and a lime wedge.

On long road trips, I sometimes sip it straight from a plastic bottle. Coke is good on long drives because it has about the right mix of sugar, water, and caffeine to stave off hunger, thirst, and drowsiness but not so much water and caffeine that I need to make frequent stops. A 16 or 20 ounce bottle is enough for several hours of driving.
I almost never drink anything straight from a metal can. It just seems wrong somehow, and I can’t explain why. I have no problem with glass or plastic bottles though.
At home - in a glass, with a shit load of ice.
Out - in a styrofoam cup, with a shit load of ice, with a straw.
Type - Pepsi if possible, RC if available, Coke as a last resort.
I rarely drink colas or other carbonated soft drinks. When I do, I prefer to drink them out of their glass bottles. That’s not often an option nowadays, but a 7 oz. or 12 oz. Coke or a bottle of Orange Crush just tastes better out of the bottle.
I do not prefer ice, but it’s necessary if you get a cup of soda from a fast food place or a tumbler at a restaurant. Generally I only drink carbonated soft drinks when I’m driving, and then only when I’m not drinking coffee or if, for some mad reason, I don’t want a nice unsweetened iced tea. So I guess the non-glass bottle option would be a plastic bottle. I virtually never drink stuff out of a can, except for those Monsters with the screw top.

On the road, it’s Coke from either from Sonic or Whataburger, so that would be styrofoam cup, crushed ice and a straw.
At home, real cane sugar Coke over Bluebell vanilla ice cream.
I like a little Cherry Coke Zero (or diet root beer) with my ice.
At work: Plastic mug filled to the top with ice. Fill rest of way with pop. Takes several pourings.
At Home: 20 ounce glass dimple stein filled with ice. Same thing.
Yeah, it waters it down and kills most of the carbonation. That’s a benefit in my book.
I get a bag of frozen sweet cherries and use those like ice cubes. As they thaw I eat them. This is best in a big heavy glass mug that got left in the freezer. And Pepsi, or maybe RC, never Coca Cola.
In a glass (yes, glass, not plastic) with lots of ice. Kind of like Chimera said. Lots of ice, then you have to make several pours as you go. I even like it watered down a bit from the melting ice.
If I’m drinking it straight up, I like my Coke Zero at room temperature.
As a mix, I like it cold with ice cubes mixed with rum (Havana Club) and a twist of lime.
Try cherry-flavored brandy in coke. It’s real good.
But don’t try it with cherry brandy. I did that once and it was a mistake.
