Help with Spanish translation

I’m looking for help translating a one-page form into Spanish, for a free health care event.

At the event, we will be distributing information and equipment so that people in need can monitor their health data and share it with their physicians. The program gives people BP cuffs, pulse oximeters, and other items as needed for free. There will be Spanish speakers there, but I need recipients to sign off that they were given items.

Here is the document in English, any help would be appreciated. I’d like the language to be as simple as possible to better serve recipients with little education (so they understand what they are signing). As you can see, the document was written by a free clinic with liability on their mind.
