Phonetically spelled Spanish phrases needed

I work for Meals on Wheels and recently I got a new client on my route who doesn’t speak English and, obviously, I don’t speak Spanish. He is always by himself when I deliver so there is no one to translate. His daughter signed him up and was supposed to be around to help but I’ve never seen her and the higher-ups are always forgetting to get in touch with her to relay messages that I need to give him.

So, what I need is someone who can write out, phonetically, some phrases that I might occasionally need to say to him.

  1. We will be closed Monday/Friday because of a holiday. Would you like a frozen meal to make up for that day.

  2. Are you ok.

  3. Do you want me to call an ambulance.

  4. Thank you for your donation.

  5. You need to have your daughter fill out the information form.

And probably:

  1. I don’t know your language. Someone wrote this out for me ( I’m thinking this might be necessary just in case he thinks I do )

I would only change 2 to:

es-TAH bee-EHN?

The phrases all translated to plain Spanish might be useful to have printed out in case showing them is easier than saying them:

  1. No atendemos el lunes/viernes porque es feriado. Quisiera comida congelada para ese dia?

  2. Esta bien?

  3. Quiere que llame a una ambulancia?

  4. Gracias por su donacion

  5. Necesito que su hija llene este documento

  6. No hablo español. Lea esto, yo no lo entiendo.
