I have to look into the LeFever article they cite as evidence of overprescription of medication (which they are using to index overdiagnosis), but the author of the piece you linked to cites another study that does not replicate LeFever’s findings.
I’m familiar with the Angold study also cited by the author, and I don’t find it compelling evidence for overprescription. Basically that study suggests that 3/4 of those who would meet criteria for ADHD actually recieved medication (a “possible” example of underdiagnosis, since 25% of those with ADHD did not recieve medication). They point to the number of kids without impairing symptoms of ADHD by parent report as evidence of the prescription of medication when criteria aren’t met. For diagnostic criteria to be met, a person has to show a certain number of behaviors of that diagnosis, AND has to show some type of impairment (e.g. problems in school, among peers, at home or at work).
The thing is, they didn’t have full teacher diagnostic report, only teacher report on the CBCL, so they couldn’t know if those kids showed impairment in school. Most of those kids DID show criterion levels of behaviors, just not impairment due to ADHD by parent report. Furthermore, 86% of those kids did in fact show impairment due to another diagnosis. Basically, that means that those parents did report that the child was experiencing some difficulty in school, at home, or among peers, but attributed that difficulty to other symptoms. That’s a pretty tough distinction to make, in my opinion.
So, the bottom line is: only 8% not meeting impairment criteria for ADHD in that study were prescribed a medication, and most of those kids did demonstrate ADHD behaviors and impairment, albeit associated by the parent with some other symptoms. I wouldn’t hang my overprescription hat on the hook of the Angold study.
