We’ve only had four presidents die of natural causes while in office, and the last one was FDR. For some reason, I can’t see that happening to McCain, but it’s just a hunch.
How many previous presidents would have been 76 years old by their first term was scheduled to end?
On the other hand, don’t forget Woodrow Wilson: didn’t die in office but severely incapacitated, I believe. And don’t most people agree that Reagan was suffering from some symptoms of Alzheimers before he left office?
Which leads me to speculate: imagine the crisis in government if McCain appeared only partially incapacitated by illness. Do we force the old man to dodder on, or do we hand over the reins of government to Ms. “I’ll try to find you some and I’ll bring 'em to ya?”

John McCain’s father died in 1981 at 70.2 years of age. John McCain’s grandfather died in 1945 at the age of 61.1 years of age. Life expectancy in 1981 was 71.1 and in 1945 it was 64.3 (by averaging 1940 and 1950) cite. Thus, his father lived 98.7% of life expectancy and his grandfather lived 95.0%. Life Expectancy for a white male is 75.7 years as of 2004 cite. John McCain is currently 72.1 years old or about 95.2% of life expectancy. Based on that, we can probably expect him to die in about the next two to three years.
However, John McCain’s mother is still alive and active at the age of 96.6. AFAICT, he has two siblings, one older and one younger, and both are still alive as well. So, I’d think his mother’s longevity also has a distinct impact that would extend his life significantly beyond what we may expect based on his paternal lineage.
Now,I’d think that the primary impact of his POW experience was stress, which would subtract a little from his life expectancy. I think the largest impact on his life would be how much stress he undertakes as president; however, I think that’s also somewhat offset by the quality of medical care that is available to the president. So, I’d guess that he could easily live to about 78-80.
So, I’d give him about a 95% chance of surviving a single term of presidency, but more like a 50% chance of surviving a second. And, just for S&G, I’ll guess he’ll die on November 11, 2015.
