What matters for the toilet paper roll is whether when you pull it, you are pulling it from the top or the bottom. So a holder that’s sideways has a top and bottom, too, unless it’s exactly next to you. If it’s if front of you, than it’s equivalent to the wall being being it to the front, and likewise for the back.
If the holder is vertical, than there is still a side closest to the wall and farther from the wall.
