I’m betting that almost all the time it happens, the poster has had multiple reply windows open, or has been perusing several threads and clicked ‘post reply’ on the wrong one.
It’s only happened to me once in my year+ on the SDMB - yesterday, of all times. But it was clearly my fault.
For the record, I suspect that was my fault, but I’ve never been quite sure. I think that was the only time it ever happened to me. I do generally use 2 browser windows so one thread can load while I read the other. It may be that I was replying to 2 threads at once and got confused.
