I’m thinking vegamite and spam.
That would be pretty salty but if the spam was grilled…
mods, technically this is about “cuisine” so I put it here but feel free to move it to IMHO if you see fit.

A haggis sandwich has to be up there.
One of those spitefully high-calorie hamburgers. You know, the ones that restaurants serve specifically because they’re bad for you.
Due to its availability, I’d say the Hardee’s Monster Thickburger would be the worst strictly from a health standpoint.
I think that would be at least edible. I eat Marmite with cheese and ham and it’s delicious.
For me the worst would be something dessert-like with mayo and other savouries. E.g. strawberry jam, raw onion, tomato and mayonnaise.
I’d try vegemite and spam. Sounds interesting … .
How about anchovies and limburger?
Actually, that sounds pretty yummy too … .
Haggis, limburger cheese and marshmallow fluff.
Or caramel/anchovy crunch ice cream.
Liver, bleu cheese and dill pickles.
Pickled herring, cheez-wiz, and carob chips on a matzoh.
Tongue with green peppers and French dressing on cinnamon swirl bread.
After making sandwiches for 5 years, I’ve decided only dirty people eat hot tuna or Reubens.
Liverwurst and grape jelly on rye.
We were once served peanut butter and green olive sandwiches.
I love peanut butter.
I love green olives.
I do not, however, love the combination.
This was provided to us by members of a church which was supporting a walkathon. All the folks in my group were dumbfounded at the concoction. Yet, evidently, SOMEONE must like this. I mean, if they were cleaning out the pantry, you’d think they’d just use the old PB, not add in olives!
I love most kinds of sandwiches no matter how healthy opr unhealthy, I can’t stand sandwiches that have stuff like oranges and meat together.
My ex liked peanut butter and mayo. Sounds nasty as all get out. Then again anything with mayo just does nothing for me.
Peanut butter and mayo isn’t that uncommon. Goes best with some Tabasco The green olive - peanut butter concoction sounds pretty nasty though.
My father loved peanut butter and a couple of thick onion slices in a sandwich.
I’ve never done that with grape jelly, specifically, but black currant jam. Liver + fruit is a pretty tasty combo. Peanut butter & onion (or I like hot peppers, too) works, as well. We just think of it as pairing with sweet here in the US, for some reason. I guess that sweet & savory combo. But it pairs just as well with savory (being pretty much savory itself, although some brands can get fairly sweet.)
For me, it’d be one of those Krispy Kreme hamburgers The Luther Burger. I don’t think it would necessarily taste bad, per se, but I don’t think my body could handle more than a bite of such greasy sugariness.
Is the OP referring to real sandwiches?
We could go on forever making up gross combinations.
I’ve seen my dad eat sardines on a sandwich with mayo, lettuce, tomato. I can’t even stand the smell. I’d vomit if I ate one.
Black pudding and Nutella.
Coyote vomit between cow-paddies. Steve-O would probably eat it.
