It’s the usual number of mods, doing their best under some quite adverse conditions.
Like everything else around here, undesirable behavior waxes and wanes. We’ll have periods of relative calm and times when people just go crazy. Sometimes we can point to something as a causative agent, sometimes it just bees that way.

I guess this example wouldn’t hurt. The OP was obviously trolling but the thread wasn’t closed until the intent was admitted on page 3. There’s some longer threads that are inhabited by Dopers who (IMO) should at least draw a warning. Perhaps they have been warned privately and if so, that’s another matter.
Sometimes trolling is “obvious”, and sometimes it is not. Sometimes one needs more evidence than just the initial post, such as follow-up comments or other initial posts by the same poster. Sometimes we do alert folks by email privately. Sometimes we read too quickly, and so don’t catch something that may be “obvious” to someone else.
The mods in some forums are reading through a LOT of posts each day. It’s real easy to skim or read casually, looking for blatant violations, rather than to read thoughtfully.
The way y’all help is by reporting posts. Even if it’s “suspected” trolling, you can report it as that way. That calls our attention to it, so it gets more than a casual reading.
