I can think of three ambigious deaths for Keanu: in the Matrix, of course, but he comes back to life because Trinity lurves him. Also in The Matrix Revolutions. And spoiler alert: Also in the fourth John Wick movie.
Sean Bean doesn’t die in evey movie, so the opposite of Sean Bean shouldn’t be someone who never dies in any movie. But then it’s not really a fair comparison, right? Most action movie protagonist are going to live through the end of the movie with any deaths being notable exceptions.

I can only find one movie (Ghosts of Mars) where Jason Statham dies and stays dead. That’s a pretty good ratio considering how many he’s killed on screen.
If the criterion is how many people your on-screen character has killed compared to number of times your character wound up deceased, it looks like Chuck Norris would be ranked highly (125 victims compared to only 4 times victimized*, according to this site).
Charles Bronson did pretty well too, with his characters accounting for 48 kills but only knocked off himself 3 times.
Yes he died on screen. His entire cameo is on YouTube. Less than 2 minutes of screen time and no lines. Still counts. So that’s 2 deaths on screen vs 726 humans killed according to this site. Pretty good ratio.
