Rolling Stone once called Billy Joel’s “River of Dreams” the worst album cover of all time, but I think that has more to do with the magazine’s ongoing feud than the action merits/demerits of the cover.
Looks Chinese, as some of the characters are different form the Japanese versions. Seems to be a pretty generic title, though.

Iron Maiden’s newest album cover looks like someone spent six minutes with Photoshop. Can’t really compare with the others in this thread, but it’s quite a step down for a band with some of the greatest covers in heavy metal history.
And ELP’s Tarkus cover is great! But Love Beach is certainly cringe-worthy…
