Contrary to popular belief, I do have a sense of decorum. I have no problem being ‘juvenile’, but I know when it’s appropriate.
I tend not to wear T-shirts in public, for a given value of ‘in public’. If I’m going down to the corner market or to the hardware store, or even if I have to run to the supermarket, I don’t mind wearing a T-shirt (and I like my T-shirts). But if I’m going to a less rural area I prefer to wear a real shirt, even if I don’t tuck it in (which I don’t when I’m wearing jeans).
That said, the T-shirt in the OP just doesn’t appeal to me. The Kool-Aid guy is mildly amusing, but the ‘vagina’ thing is about as funny as someone saying ‘poo-poo’ for no reason in a public place. It’s not clever. It’s not the kind of shirt I would wear at all, let alone in public.
