We had a three year period where the only pizza delivery option was Pizza Hut. Had a couple bad experiences. So I would drive elsewhere and pick one up to take home.
I happened to see a Papa Johns car in my neighborhood and started using them. We call at least once every 2 weeks. Really… every 10 days is closer. 
but for the poll I’ll answer 2 weeks.
How about you and your family? How often do you get Pizza Delivery from some company?
Don’t include pickups at the restaurant.
I share a house with my brother, his wife, and her 4 kids from a previous marriage. There’s usually delivery pizza a good 4 days a week. The oldest daughter is 17, and has a full time job (until school starts again, then her hours are reduced). With the 14 year old twin boys at sports practice, and everyone else’s work schedule, they’re almost never together long enough to have a family meal, except for holidays.
I voted Never. We’re retired. We like to get out. We go out to a neighborhood Italian place that makes wonderful pizza. Sure beats the heck out of those cheese-stuck-to-the-cardboard things we used to have delivered.
We used to do frozen pizza when our daughters where at home. Now we splurge with fresh pizza. 
A lot better.

Hot, fresh, meltingly delicious pizza that the Gods Themselves would crave can be delivered from less than two blocks away.
I’m shocked that I only indulge every two weeks or so.
We pick up a pizza every month or so. I just drive the 5 minutes to the shop instead of asking for delivery.

Little Pianola and Little Banjo (ages 26 and 20 now) just visited from their Midwestern colleges (Chicago and Missouri) and insisted of Brooklyn pizza for dinner three times a week. Also for lunch or midnight snacks. Primarily Nonna style.
The last time I had any food delivered was in the mid-'80s, when I was without a car, living in a place where I needed one. So I voted “never,” as “occasionally” would have been misleading.
I live too far out in the boondocks for pizza delivery. Make my own (excellent if I do say so myself) homemade, including from-scratch crust, every few weeks. Don’t miss delivery at all. 
It’s probably somewhere between once a year to once every two years, so I voted “occasionally.” I do pick up pizza more often than that. My favorite pizza places either don’t deliver or are out of my area, so I have to get my ass out there to get it. We probably have carryout pizza at home four or so times a year.
The two to three good places we order from regularly either don’t do delivery or we are out of their area. So we do a lot of carry-out pizza.
If we want delivery it’s Pizza Hut, Dominos, or Papa Johns. So pretty much never.
Depending on whats going on, we’ll order pizza every now and then. Sometimes more than others. I guess the average would be about every 3 weeks. That’s if we’re just talking about pizza. SOMEthing always seems to get delivered every week or more when the wife’s friends come over. Usually wings or burgers or some such.
I voted never. We do eat restaurant pizza (and other meals) at home fairly frequently, but I usually just walk or drive and pick it up.
We haven’t had delivery pizza in many many years, so I put never. But we do have carryout from Papa Murphy’s maybe 2-4 times a month? It’s pretty dang good, relatively inexpensive, and I don’t mind schlepping out the 10 minute drive to pick it up.

I don’t eat pizza that often anymore, and most delivery pizzas aren’t all that good. There are a couple of pizza joints in the area that have a good quality product, but they don’t deliver.
Well in that case, never.
As a family of just me, it’s more economic to pick up.
Very rarely. Despite the fact that I live less than a mile from the state Capitol building, I’m somehow just outside the delivery radii of any of the major pizza chains. There are two local places that deliver to me, but one is overpriced and the other has a standing carry-out special of large 2-topping pie for $8.99, which is nearly half the price of having that same pizza delivered.
Really, these days when I’m in the mood for pizza, I usually just swing by Mod and get a pizza-for-one with some crazy combination of toppings based on whatever suits me at the moment.
I used to pick it up when I had teenage kids but can’t recall ever having it delivered. I ordered some delivered to my father’s place while staying there to care for him, and unable to leave him alone to pick it up, but that is the only home delivery I can ever recall.
So once in a lifetime I guess.
