Is "rinsed penis" really that much of an insult?

I mean if you’re gonna insult someone’s genitalia wouldn’t something like puss filled herpes infested penis be more appropriate? And what exactly is a rinsed penis anyway? Does he use a bidet - is meant to hint that he’s out of touch, elite “european”?

The reason the whole santorum thing worked was because that’s gross. You can’t just string vulgar words together that sound like someone’s name and have it be an insult.
