A witness told police the black pickup truck was swerving in and out of its lane on a two-lane rural highway before crashing head-on with the van, Ruvalcaba said. “At this point, we don’t know whether alcohol or drugs played a factor," he said.

On the other hand another article referred to the pickup driver as elderly–so it could be a medical issue–and not appropriate for this list.

A 16-year old has killed two adults and wounded two others in the extremely tiny (800 residents) and extremely remote village of Point Hope, Alaska. Assault, rape and murder have been problematic in remote villages for a long time. In many villages there is no police presence, but in this case the boy’s father brought him to the local station.
