As to the OP, I nominate the dance number with the gas that turned the family inside-out.
For me, it’s a tie between the football place kicker’s broken leg flying off, and Homer getting nearly disemboweled by the badger in the doghouse.
Kirk Van Houten’s arm getting sliced off by a wire while he was driving his car.
And much of the “Homer’s thumb gets cut off” segment on the “Run Lisa Run” episode was pretty nasty.
Homer eating that moldy, decaying, hero sandwich was pretty nasty.

Bart tries to create a prince from a frog, but creates a half-dead monstrosity that isn’t quite either.
When Homer gets his jaw wired shut and sucks the bite of steak through his nose. I don’t know why, but that always turns my stomach.
For me it was Homer’s eyes after laser surgery. He refuses the eye drops, and immediately a baklava-like crust forms over his eyes. It was truly the stuff of nightmares. I thought I had gotten over it, but I’m about to have a seizure right now as I type.
When Homer’s face is sucked in by a power vacuum, leaving one eye severely swollen.
When Springfield is destroyed by a neutron bomb, and a few survivors are turned into mutants, and Wiggum’s eye falls out.
Any scene with Mr. Burns naked. :::shudder::::
When the family went to Florida for spring break: Homer’s suggestion that Bart and Lisa would get married.
Willie, on briefly owning Santa’s Little Helper: “I also 'ate the mess he made on me carpet. …Ye heard me!”

Comic Book Guy and Agnes in the motel room bed-- oh my god.
Funny disgusting moment- Simpsons in Japan- the camera wired to the super high-tech toilet which had its own channel on the rooms’ Big Screen TV.
Bart: Hey look there’s dad.
Homer: Ahhhhhhhh.
Family (seeing the image off screen): YAHHHHHHHHHHHHHHHHHH!
Man I almost busted a gut on that one.
From the episode where sugar was band:
Homer: Look, a sticky spot (procedes to lick the floor)
Apu: Homer, that is dried blood and vapor rub you are licking.
Homer: Somehow, I knew that.
And then at the end of the conversation, he goes back to licking it.
“Bacon that Butter boy. Bacon it.”
Homer goes to spy on Apu with a hat camera, and accidentally drops the hat on the floor when he leaves. It then films Apu dropping a hot dog from the wiener roaster on the floor; it rolls in close to the camera lens and we see it all covered with used band-aids, hair and dead roaches. Apu picks it up and replaces it in the roaster.
The New York episode when the garbage is flying off the garbage truck in front of the car and hitting Homer in the face. SPLAT!
Also, the Garbage Commissioner one when the posium blows up and hits Mayor Quimby in the mouth and his mouth goes all green. YUCK!
When Homer’s spring-in-a-can gag backfires and gets lodged in people’s eyes. I usually don’t get grossed out, but I always feel a little “eegh…” when I see that.
mobo85… the worst part was when Lenny & Moe (?) got their eye-springs tangled together.
And didn’t Lenny get a puncture would to his head from a high-speed coin, or something?
Homer gushing blood after cutting his hand on Moe’s jukebox was pretty nasty.
But the winner is Kirk Van Houten getting his arm sliced off. I don’t care that they showed him healing at the end.
BTW, I should apologize for being the third person to employ the oxymoron “pretty nasty.”
Also, I’ll add that “rat milk” is definitely a runner-up to the arm-slicing. These are things that should not be.
