Blurf.
I did another rare work-week bambilance shift last night, and I passed a personal milestone of sorts. For the first time, and working on my own, I used my -Intermediate skills on a patient.
I’ve been using them ever since I got the card, but until now, it’s always been under the direct supervision of another -I or paramedic, as a probation/internship type of deal. I have to log 8 calls of the appropriate skill level, and have them signed off by the supervisory EMT that I knew what I was doing and acted appropriately before I can play under my own power. I’m up to 5.
So, last night I drew a shift, and I had no scheduled partner. Sue, my potential adoptive grandmother, was going to answer from home to cover. She’s not even an EMT, so all she can do is drive.
The call was for a woman with severe leg pain. Sue met me at the patient’s house, and she had a long history of sciatic nerve problems. I did a quick exam, and this was pretty much a classic case; she was hurting in her left leg from her butt to her toes. I took her vitals, and just about had a calf on the spot, because her blood pressure was 230/110. If she had a stroke, her head would have popped like a ripe zit.
She was a big girl, and it took some effort to get her out of the house. We loaded her up, and I told Sue that I wanted to consult with our Medical Director on the BP issue before we left.
“Hmmm. Not much you can do for her blood pressure. Administer 30 mg of Toradol.”
“Copy. Administer 30 of Toradol.” Toradol is a liquid NSAID, in the same drug family as aspirin and naprosyn/Aleve.
The doctor had given me a reasonable and valid medical order, I explained the need for his permission, because this particular doctor is personally the one who determines when I can act on my own. Wrote everything down for the report, and I gave the lady her shot.
When I checked her vitals about 10 minutes later, her BP had dropped significantly. She must have been hurting BAAAAD to have that kind of response.
I did everything right, and this wasn’t that big of a deal, anyways. The real fun will be when the Chief and the Ass Chief read the report and see what I’ve done, 'cuz they’ll freak and try to give me hell. That’ll be a bit of a showdown, but I’m covered.
