Any time Mussolini is used in a joke, it’s to refer to things running on time or being made to run on time. There really is no other humorous purpose for invoking the name.
In this joke, Sicks Ate is playing upon this well-known Mussolini trait to imply that his wife, the most regular woman, could never, in fact, have a period off schedule.
