I think the point is that he’s putting on weight pretty significantly, in a short time. There’s much speculation that he’s heavier because he’s retaining fluid (see the swollen ankles from last year, which the White House says is due to “chronic venous insufficiency”), which is never a sign of a good thing.
Drugmaker Eli Lily and the FDA granted a single “compassionate use” exception for the experimental drug Retatrutide which targets GLP-1, GIP and glucagon. Speculation has it that Trump was the one who got this exception (as they were able to specify it was for a 79 year old male patient, matching Trump’s age at the time). So there he is on this experimental super-powered fat buster (apparently 100% of the people in previous trials lost at least 5% of their weight and 83% lost 15% of their weight or more) and he’s getting bigger… and bigger. If he’s on this drug, he’d be getting weekly doses. And unlike the Leqembi injections that I believe he is getting monthly to knock plaque off his brain, Retatrutide is just delivered by auto-injector pen into fatty tissue (so for him, basically anywhere). As long as he has some sycophantic young assistant who can keep track of his medication schedule he should be losing all kinds of weight right now.
Well, okay, but the linked article didn’t say any of that. Is there a reputable site that talks about the implications of his weight gain?
I’d try increasing the dose.
Here’s the study documenting the 100% effectiveness on the patients who weren’t taking the placebo. It doesn’t talk about the implications of weight gain because nobody gained weight. But you know Trump, he’s always got to be special.
Also, one wonders about side effects. I had some very unpleasant side effects from Mounjaro (the doctor had to back my dosage back down, and now we’re sneaking up on the higher dosage a little at a time to see if we can make it work). I can’t imagine Trump would put up with nausea and vomiting, even if he could keep it private.
It seems to me the article was less about doctor’s concerns over his weight and more about his [false] statement that he would never call anybody fat, along with a bunch of quotes of him calling people fat.

Again, that wasn’t mentioned in the article that was cited.
Relatively sudden weigh gain or loss is usually bad, but how bad depends on the reason. People here seem to be assuming it’s this drug, which is possible, but I don’t know that the dates line up all that well. Problems with fluid buildup seem more likely to me, but it could be a fast-growing tumor or combination of several factors.
All we can do until somebody leaks better information is look forward to the autopsy report.

Epistemic Crisis is one of my favorite Youtube channels when it comes to pushing pins into a Trump voodoo doll on a daily basis. He’s a physical therapist with a doctorate in his field and he’s seen plenty of patients just like Trump. He puts out a new video every day about Trump’s ongoing health issues. Today’s was interesting, he believes the big bulge at the back of Trump’s suit might be an exoskeleton for helping him move. There is an example of what one looks like in the video. The other option is that Trump might have a nephrostomy tube, which does direct drainage from the kidneys into an external reservoir. Two minute video here:
